How You Can Provide Feedback or Make a Complaint

In Person: Paper-based Feedback and Complaint Forms are included in your Welcome Pack and are also available to download here. You can hand completed forms to any Flourish United team member.

All information you provide will be managed in confidence and in accordance with our Privacy and Confidentiality Policy.

If you wish to give feedback, you are welcome to remain anonymous. However, please note that we may not be able to provide an update or seek further input from you about your feedback if you choose to remain anonymous.

Once we receive your feedback, it will be reviewed within 48 hours, and any necessary follow-up actions will begin as part of our feedback resolution process. If your feedback requires a response, we will contact you to discuss it further.
